Traveling to Kindergarten FORMS
The Traveling to Kindergarten process is meant to facilitate a connection between a child's preschool teachers/caregivers within a child care program and the kindergarten teacher within the school district for that child. Utilizing the Traveling to Kindergarten Form, the preschool teacher/caregiver completes the document with the parent at an arranged parent/teacher conference. A copy is made for the child's file. The document and a kindergarten checklist is given to the parent to take to kindergarten registration.

- Download all three documents (see below) - Traveling to Kindergarten Document, Parent Checklist and the Mutual Agreement
- A member of the REACHigher Birth - 2nd Grade workgroup would be happy to provide a presentation on the use of the Traveling to Kindergarten document. Please feel free to contact us through the Contact Us section of the website.
- Complete the Mutual Agreement document and fax or mail to the information provided on the document
- Determine the registration/start dates for the school districts you feed children into - this will determine when you plan your parent/teacher conferences for the spring.
- Set parent/teacher conference and complete the Traveling to Kindergarten document with the parent (including signatures). Make a copy for the child's file within your program and give the original to the parent to take to kindergarten registration along with the parent checklist which lists the items needed for registering their child.
- Some optional things to consider - plan a field trip for children going to kindergarten to visit a kindergarten classroom, set up a call or visit with the kindergarten teachers your program feeds into(you can get information about what they expect kids to know coming into kindergarten, etc.)
- Use the process to help inform instruction, parental connections and ongoing connections with local kindergarten teachers for the next year.
